El fentanilo es una droga opioide de moda que causa graves problemas de salud en México, Estados Unidos, Canadá y muchos otros países. Actualmente, se vive una crisis sanitaria debido al gran impacto que ha causado el tráfico ilegal de fentanilo. Es el opioide más antiguo de origen sintético, es de 50 a 100 veces más potente que la morfina y su uso principal se da en la medicina, como anestésico y tratamiento en dolor agudo. El uso incorrecto de fentanilo en la población, ha incrementado el número de muertes registradas por sobredosis, y aunque las estadísticas no mencionan exactamente el número de decesos por fentanilo, se atribuye el aumento al uso de esta droga.
¿Qué es el fentanilo?
El fentanilo es un agonista (sustancia que es capaz de unirse a un receptor celular y provocar una acción determinada en la célula generalmente similar a la producida por una sustancia fisiológica.) opioide, sintético muy potente. Los opioides como el fentanilo pertenecen a un grupo de medicamentos analgésicos derivados del opio que son utilizados para el tratamiento del dolor, estos medicamentos tienen la capacidad de unirse con los receptores opioides que están presentes en las células del cuerpo y en el sistema nervioso central.1
Los receptores opioides que están presentes en el organismo son estimulados por péptidos endógenos como endorfinas, encefalinas y dinorfinas, es decir sustancias producidas por el propio organismo en respuesta a estímulos dolorosos provenientes del medio.
El fentanilo se une a un receptor opioide presente en el interior o la superficie de las células del cuerpo y produce la misma reacción que las endorfinas, encefalinas y dinorfinas que se producen de manera natural en el organismo, pero de una manera más potente desencadenando una reacción intensa.
Estos receptores opioides son principalmente los receptores mu (µ), que se encuentran en su mayoría en el tronco encefálico y el tálamo medial. Los receptores mu (µ) son responsables de la analgesia supraespinal, depresión respiratoria, euforia, sedación, disminución de la motilidad gastrointestinal y generan dependencia.
Los subtipos incluyen mu (µ)1 y mu (µ) 2
mu µ1: Provoca analgesia, euforia y sensación de bienestar
mu µ2: Provoca depresión respiratoria, prurito, liberación de prolactina, dependencia, anorexia y sedación2-3.
Debido a las sensaciones de bienestar y euforia que el fentanilo provoca, es altamente adictivo, pero el efecto de sedación y depresión respiratoria que provoca, es responsable de que la gente muera utilizando este fármaco de manera ilegal. A diferencia del uso en un escenario controlado donde exista una fuente de oxígeno suplementario, equipo para ventilar y personal de la salud especializado en manejo de la vía aérea para que cuando aparezcan estos efectos de sedación y depresión respiratoria no haya complicaciones en el paciente2-3.
Fentanilo se encuentra disponible para su uso en forma parenteral, transdérmica y vía oral, solo debe ser utilizado bajo indicaciones específicas del personal de salud experto en su uso como lo serían los anestesiólogos, algólogos y paliativistas. El uso más común del fentanilo es por vía intravenosa, manejado por el anestesiólogo ya que es el opioide más utilizado en cirugías para control del dolor transoperatorio. En México es legal su uso de manera hospitalaria y de forma ambulatoria puede ser comprado con un recetario del sistema de recetarios electrónicos para medicamentos de Fracción I. Esta Plataforma permite a los médicos tramitar recetarios especiales para estupefacientes y a las farmacias validarlas para su dispensación.
Situación del fentanilo a nivel nacional y mundial
El consumo de sustancias psicoactivas presentará variaciones con tendencia al aumento y este aumento viene desde años atrás, como lo muestra la Encuesta Nacional de Consumo de Drogas, Alcohol y Tabaco 2016-2017 (ENCODAT) que en la población de entre 12 y 65 años el consumo de cualquier tipo de droga fue de 2.9% lo que representa a 2.5 millones de personas que consumieron alguna droga y aunque esta encuesta no reporta casos activos de consumo de fentanilo existen otros estudios como “Cuqueando la chiva” o “Caracterización de los servicios para personas que consumen heroína, fentanilo y otros opioides en la frontera norte”, mostraron un panorama muy distinto sobre el consumo de opioides en la frontera con los Estados Unidos, específicamente en las entidades de Baja California, Sonora y Chihuahua.4
Situaciones globales como la pandemia por COVID-19 y las medidas de distanciamiento social que se implementaron para controlar la pandemia en nuestro país, además del desempleo que se originó, desencadenó una crisis de manera paralela, y es que los cambios en la salud mental hicieron que este incremento el cual ya hemos planteado, se potencializara aún más.
México cuenta con la Red Nacional de Atención a las Adicciones, que ofrece tratamiento de manera ambulatoria. Durante el año 2020, esta Red proporcionó atención a 92,989 personas que solicitaron ayuda debido a problemas asociados al consumo de sustancias ilegales y fue durante este mismo año que en México la Comisión Nacional contra las Adicciones (CONADIC) atribuyó un total de 1,735 muertes por sobredosis, ninguna de ellas específica como causa al fentanilo, sin embargo, este número podría estar infra estimado, ya que no siempre se da seguimiento a las causas de muertes.5-6.
Durante el mismo 2020 también se realizó un seguimiento estrecho de los datos proporcionados para identificar el uso de fentanilo entre personas que solicitaron atención en los centros de ayuda públicos, privados y organizaciones civiles, entre 2013 al 2020 y se detectaron un total de 122 casos activos (Tabla 1).
Datos estadísticos
En los centros de adicciones estudiados por el CONADIC también se encontró que el número de personas que habían consumido fentanilo era de 24 entre el 2013 y 2018 y pasó a ser de 100 registrados en los dos últimos años, lo cual muestra que la población mexicana está cada vez más expuesta al fentanilo (Figura 1).7
Pero este problema no es exclusivo de México, ya que en Estados Unidos y Canadá también han aumentado el número de casos de muertes registradas por sobredosis. El Gobierno de Estados Unidos no lleva un registro de muertes específico por cada droga, el Centro Nacional de Estadísticas de Salud (NCHS) y el Centro para el Control y la Prevención de Enfermedades (CDC) recopila información sobre las muertes atribuidas a sobredosis por uso de drogas ilegales, así como de opioides recetados hasta el año 2019, donde también resalta un notable incremento.
Insight Crime, publicó que en Estados Unidos el número de muertes atribuidas a una sobredosis durante el año 2020 rompió récord y alcanzó las 93,331 muertes y en el 2021 las cifras seguían en aumento.8 (Figura 2)
En Canadá el problema también se intensificó durante la pandemia de COVID-19, muchas comunidades de todo Canadá han dado a conocer cifras récord de muertes, llamadas de emergencia y aumento de hospitalizaciones relacionadas con el abuso de opioides. 9
Estos contextos y datos en el aumento por sobredosis como resultado de la crisis paralela por la pandemia de COVID-19, muestran la importancia de que los servicios esenciales deben estar siempre al alcance, además de promover el cuidado mental implementando el uso de tecnologías para informar a la población acerca de éste y muchos temas más.
